Job Description: Clinical Pharmacist

We are currently seeking a knowledgeable and compassionate Clinical Pharmacist to join our healthcare team. As a Clinical Pharmacist, you will play a vital role in providing pharmaceutical care to patients in clinical settings, collaborating with healthcare providers, and optimizing medication therapy to improve patient outcomes. Responsibilities:
• Medication Management: Review medication orders, patient histories, and laboratory data to assess medication appropriateness, effectiveness, and safety. Collaborate with prescribers to optimize medication regimens, adjust dosages, and resolve medication-related problems.
• Clinical Consultation: Provide clinical consultation and drug information to healthcare providers, patients, and caregivers. Answer drug-related inquiries, offer evidence-based recommendations, and facilitate medication management discussions to ensure informed decision-making.
• Medication Therapy Monitoring: Monitor patient response to medication therapy, including therapeutic outcomes, adverse effects, and drug interactions. Use clinical judgment and evidence-based guidelines to assess treatment effectiveness and modify therapy as needed to achieve desired outcomes.
• Patient Education: Counsel patients and caregivers on medication use, including dosage instructions, administration techniques, and potential side effects. Educate patients about medication adherence, drug interactions, and lifestyle modifications to promote safe and effective medication use.
• Clinical Documentation: Document clinical interventions, medication recommendations, and patient counseling activities in the electronic health record (EHR) or other designated systems. Maintain accurate and comprehensive records of pharmacy interventions and patient interactions.
• Interdisciplinary Collaboration: Collaborate with interdisciplinary healthcare teams, including physicians, nurses, pharmacists, and other allied health professionals, to coordinate patient care, resolve medication-related issues, and achieve optimal therapeutic outcomes.
• Quality Improvement: Participate in quality improvement initiatives, medication safety projects, and performance improvement activities to enhance the quality and safety of medication use. Contribute to medication-related policies, protocols, and clinical guidelines.

Requirements:
• Education: Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program.
• Licensure/Certification: Current licensure as a Pharmacist (RPh). Board Certification in a pharmacy specialty area (e.g., Pharmacotherapy, Ambulatory Care) preferred but not required.
• Experience: Previous experience in clinical pharmacy practice preferred. Familiarity with medication therapy management, pharmacokinetics, and pharmacodynamics.
• Clinical Skills: Strong clinical assessment and decision-making skills. Ability to interpret medical literature, evaluate drug information, and apply evidence-based guidelines to clinical practice.
• Communication: Excellent communication and interpersonal skills. Ability to communicate effectively with patients, caregivers, and healthcare providers.
