Serving nearly 55,000 individuals each year, the LAA offers Atlanta area Latinos an extensive range of evidence-based services, programs, and supports to help them fully and successfully integrate into the community. Our focus areas & programs include Family well-being; Economic Empowerment; Immigration Legal Services; Youth Education and Advocacy.

Department Goal: The Family Wellbeing department aims to empower Latino individuals and families, fostering self-sufficiency, health, and stability. We address food insecurity, mitigate social determinants of health, prevent homelessness, and provide crisis resources.

Position Primary Function & Purpose:

As a crucial member of the Family Wellbeing team at The LAA, the Lifestyle Coach/ Promotora de Salud plays a pivotal role in implementing/facilitating the CDC-approved lifestyle change program- Prevent T2, a CDC-recognized lifestyle change program designed to help pre-diabetic people cut their risk in half for developing type 2 diabetes. This program is also referred to as the National Diabetes Prevention Program (NDPP). This part-time, contract position focuses on empowering low-income Latino adults, ages 18-70, diagnosed with or at high risk for Type 2 diabetes, to adopt and sustain healthy eating and physical activity habits. The role involves delivering culturally relevant services, such as Zumba and cooking traditions, facilitating mental health education sessions, and utilizing app-based monitoring for holistic diabetes prevention. The Lifestyle Coach fosters a motivating and supportive environment, fosters community engagement, and collaborates with health and social service providers. This position aligns with LAA's mission to empower Latinos to thrive by addressing social determinants of health and promoting holistic well-being.

Program/Grant specific Requirements to be met/completed upon hire: This position has state and federal requirements that need to be met. The candidate will participate in, and complete, the Lifestyle Coach Training. All training time and registration fees will be covered by the Hispanic Federation.

Qualifications/Skills Knowledge: People who have been trained to deliver the required curriculum content and possess the skills, knowledge, and qualities listed below are eligible to be lifestyle coaches. Lifestyle coaches may have credentials (e.g., RD, RN), but credentials are not required.

Skills, knowledge and qualities

After receiving program training, lifestyle coaches should be proficient in the following areas:
• Organizing program materials and delivering the program with adherence to a CDC approved curriculum Facilitating groups to optimize social interaction, shared learning, and group cohesion Understanding and overseeing participant safety-related issues with respect to program delivery

In addition, lifestyle coaches should demonstrate the following skills, knowledge and qualities:
• Ability to guide behavior change efforts in others without prescribing personal actions or solutions, so that participants increase their self-confidence and capacity to make and sustain positive lifestyle changes Ability to communicate empathy for participants, who will likely experience difficulty and frustration at times when trying to adopt and sustain healthy lifestyle behavior changes and who may be unlike the lifestyle coach in terms of weight status and level of commitment to living a healthy lifestyle Ability to build strong relationships with individuals and build community within a group Knowledge of basic health, nutrition, and fitness principles Knowledge of the principles of behavior change, including motivational interviewing techniques Commitment to the mission of the organization that is offering the program Flexibility to work with people from all walks of life Strong interpersonal and communication skills
• Attention to detail and data collection Knowledge of and/or is part of community served
